Foundations Group of MN Inc offers homemaking services for Minnesota Residents as a certified 245D Waiver Services Provider. Some of the homemaker services we provide include general house cleaning and common household activities.

ICLS is a bundled service that includes six service components. ICLS services offer assistance and support for older adults who need reminders, cues, intermittent/moderate supervision or physical assistance to remain in their own homes.

Services for people who live in their own home or family home that provide support and/or training in the community living service categories listed in the covered services section. There are three types of individualized home support services; without training, with training, with family training.

Personal care assistance (PCA) services provide assistance and support for persons with disabilities, living independently in the community. This includes the elderly and others with special health care needs. PCA services are provided in the Minnesota Health Care Programs (MHCP) member’s home or in the community when normal life activities take him or her outside the home.

Foundations Group of MN is a certified and licensed respite care provider in Minnesota providing short term care to a person when their primary caregiver is absent or needs relief.
