What are PCA Services?

Personal care assistance which is commonly referred to by the acronym (PCA) services provide assistance and support for persons with disabilities, living independently in the community. These services are catered to the elderly and others with special health care needs. PCA services are provided in the Minnesota Health Care Programs (MHCP) member’s home or in the community when normal life activities take him or her outside the home. Foundations Group of MN Inc is an authorized provider of PCA services in the following Minnesota Counties:

  • Northern Washington County
  • Chisago County
  • Isanti County
  • Eastern Anoka County

Eligible Members

MHCP members with eligibility for one of the following MHCP programs are entitled to an assessment for PCA services to determine eligibility:

  • Alternative Care Program (AC)
  • Emergency (EH)
  • MinnesotaCare for children under 21 & pregnant women (KK)
  • MinnesotaCare for children under 21 & pregnant women (LL)
  • Medical Assistance (MA)
  • State-funded MA (NM)
  • Refugee (RM)

Members not eligible to receive PCA services are those eligible for one of the following programs:

  • MinnesotaCare for parents, caretakers & adults without children (BB)
  • MinnesotaCare for parents, caretakers & adults without children (FF)
  • Minnesota Family Planning Program (MFPP)
  • HIV/AIDS (HH)
  • MinnesotaCare for parents, caretakers & adults without children (JJ)
  • Qualified Medicare Beneficiary (QM)
