Foundations Group of MN Inc accepts medical assistance payments once the individual(s) receiving the services have been approved through the county in which they live. Each county has its own assessment process to determine whether or not individuals qualify to receive medical assistance.
If a PMAP is chosen, Foundations Group of MN Inc is currently contracted to accept payment from the following carriers for PCA services.
